Referral Doctors

Working together helps us all offer local pets access to exemplary medical care. We are pleased to offer both after-hours and daytime emergency care. It is both an honor and a privilege to care for your beloved patients and we do not take these responsibilities lightly. You can be sure that we will offer your referred clients the level of care they need to have the best chance at a positive outcome.

Communication is incredibly important to each member of the SportsVet staff and we will keep you informed of any developments with your patient's diagnosis and progress while we are treating them. We provide you with updates, detailed treatment summaries, and test results. SportsVet wants to work as a team with your office so we can conquer your patient's most challenging health needs.

If you transfer a pet to our facility for overnight or weekend care, we will work closely with you to make sure you are up to date on the pet's medical condition. When it's time to transfer back to your facility, we will make sure the pet is set to transfer safely and provide all medical records to you for seamless continued care.

Having as much information about a transferring case as possible, helps us to pick up exactly where you left off. When sending a patient, please fax or email the following form:

Testimonials

Feedback from our clients

  • "SportsVet Animal Medical Center was the best vet I have ever had. I live in Minneapolis now and not only are all of the vets very expensive, they do not really seem to care in the same way Dr. Jacobs did."
  • "Dr. Jacobs and his staff are AMAZING. Every time I called, which was often (multiple times a week), they were happy to speak with me. They even sent me video of her during her underwater treadmill therapy. 4.5 years later, while some of her other joints struggle, her knee is perfect."
  • "Dixie had her TPLO surgery on Jan. 10th and went for her first water therapy yesterday. She is doing great. Getting stronger and using her leg more everyday. Everyone at SportsVet is soooo supportive and caring. Thank you again so much!!!!"
  • "I’ve never seen a vet be so caring and compassionate to not only the animals, but the animals owners as well. We think so highly of Dr. Jacobs and his staff that we travel and hour and a half for all our vet needs."
